Tefillin Information

I want to know the details about my tefillin, but the ink is faded so it’s hard for me to tell. I can tell that the tefillin are Dakot, that the knot is Pnimah, and that the head knot is Dalet (didn’t need the page to tell me that).

What I can’t tell, is what the two on the top left are and the Parshiot. I’m pretty sure the parshiot stand for Arizal, but I’m not sure. I also can’t read Ktav, so I can’t tell what type of Retzuot they are.
