Questions about the Jewish naming process (Hebrew names)?

I know these names are typically given during the newborn stage, and Jews by choice can choose one for themselves. I do have a few questions about the process that I haven’t been able to find online.

  1. What if a child’s first name is already of Hebrew origin? Are they still typically given a second Hebrew name? Would there still be a ceremony?
  2. Is the Hebrew name included on the birth certificate (or any other official document)? If so, do Jews by choice typically change their birth certificate to include this Hebrew name?
  3. Are there any set rules on what the name can/cannot be?
