Can someone explain Hebrew names?

Hi everyone! I’m confused about the formatting of Hebrew names. I keep seeing different formats and I’m unsure of which is the correct way to write one or if different denominations have different ways of writing them. I’ll be giving some examples of what I’ve seen in a transliteration format so you can see what I mean:

Rachel bat Sara

Chanah bat Yosef v’ Sara

Shlomo ben Yosef

Abraham ben Sara

Rachel bat Yosef

As you can see from these examples, I’ve seen names that use both parents names, or only the mother’s or only the father’s. This doesn’t seem to depend on the gender of the child either. Let me know if you can help me clear it up!

Edit: Grammar and clarification
